OAE Hearing Screening Tracking and Monitoring Tools

Early childhood program coordinators and staff often need help in documenting hearing screening outcomes, tracking children who need follow-up, and monitoring screening program quality. The downloadable forms and online tools available here have been designed specifically to meet these needs.

Start by viewing and printing the Tracking Tools Overview [PDF]. This overview provides examples of the tools and explains how they are used together to create a comprehensive tracking and monitoring system. Next, access and use each tool below in your own screening program.

Step 1: Documenting OAE Hearing Screening and Diagnostic Outcomes.

These forms make it easy to record ear-specific outcomes for each child’s health record while guiding screeners through the protocol and “next steps” needed.

Tools

OAE Screening Form [PDF]. Print one copy of the form for every child who will be screened and record outcomes as screening is conducted. The completed form can be filed as part of the child’s health record and can also be copied and transmitted to the health care provider or audiologist if a particular child requires follow-up evaluation after screening.

Diagnostic Follow-up Form [PDF]. Print one copy of the form for every child who needs follow-up after OAE screening [typically less than 10% of the total screened]. As prompted by the protocol, obtain diagnostic outcome information from the health care provider and/or pediatric audiologist.

Step 2: Tracking OAE Screening and Follow-up Diagnostic Activities

This log and summary sheet helps you keep track of which children have completed the screening and which need follow-up. It also assists you in periodically summarizing screening outcomes.

Tools

OAE Screening and Diagnostic Log [PDF]. Print enough copies so that you can record one child's name per line [20 names per page]. Chart each child’s progress through the protocol.

Summary Sheet of OAE Screening & Diagnostic Outcomes [PDF]. Print one copy; periodically use it to tabulate the results of the Log. Use this summary data in annual reports, during site review visits and as evidence of compliance with performance standards.

Step 3: Monitoring Outcomes using the Online OAE Screening & Tracking Self-Monitoring Tool

This online tool is a quick way to obtain periodic feedback about your screening and follow-up activities, monitor the quality of your screening program, determine if technical assistance if needed, and highlight important outcomes from your screening efforts.

Sign up to use the Online Self-Monitoring Tool. After you sign up (once), you will be reminded periodically (approximately 6 times per year) to enter totals from your Summary Sheet into the Online OAE Screening & Tracking Self-monitoring Tool to obtain immediate feedback. Be sure to check your email after using the Online Tool for your automatic analysis. If you are submitting data as a participant in the ECHO initiative, the audiologist or team member who is assisting you will also receive a summary of your data.
