- One hearing aid per ear every 36 months. One Cochlear Implant per ear with internal replacement as medically or audiologically necessary. An external speech processor and controller with necessary components replacement every 36 months.

- 18 Years or under
Amount:
- Cost of equipment and the cost of related services and supplies
ACA Coverage:
- $1,000 every 36 months
